What You’ll Do: In this role, you’ll manage a diverse caseload, focusing on assessing and treating patients in their homes. You will be part of the Urgent Community Response Team, making a significant difference to patients’ recovery journeys. You’ll also collaborate with other healthcare professionals, such as nurses and occupational therapists, to create comprehensive care plans.
