Job Description:
Hunter AHP Resourcing is seeking a skilled and motivated Independent Prescriber to join a Primary Care Network (PCN) in a flexible, remote capacity. This is an excellent opportunity for an experienced Independent Prescriber to work autonomously, providing high-quality patient care while enjoying the flexibility of remote working.

As an Independent Prescriber, you will be responsible for assessing, diagnosing, and prescribing treatment for patients within a community setting. You will work closely with the multidisciplinary team in the PCN to manage long-term conditions, offer specialist advice, and ensure the highest standards of care.

Key Responsibilities:

• Conduct remote consultations with patients, assessing their healthcare needs
• Prescribe medication and treatment plans in line with clinical guidelines
• Review and monitor ongoing care for patients with long-term conditions
• Work closely with GPs, nurses, and other healthcare professionals to provide holistic patient care
• Provide expert advice on medication management and pharmacological treatments
• Maintain accurate patient records and ensure confidentiality and compliance with regulations
• Offer guidance and support to patients regarding lifestyle, medication, and health improvement
• Contribute to quality improvement initiatives and support service delivery

Requirements:

• Registered Independent Prescriber (NMC, GPhC, or GMC)
• Relevant clinical qualifications and experience in prescribing
• Experience working in a Primary Care or PCN setting (preferred)
• Strong knowledge of chronic disease management and medication therapy
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a remote multidisciplinary team
• High level of professional and ethical standards
• IT proficiency and comfortable using digital platforms for remote consultations
