Position: Locum Independent Prescriber Pharmacist (Maternity Leave Cover)

Start Date: ASAP
Duration: Up to 12 months
Working Hours:

• 8:00 am – 4:00 pm
• 9:00 am – 5:00 pm
• 10:00 am – 6:00 pm
  (Part-time or full-time – 2, 3, 4, or 5 days per week)
  
  
• Pay Rate: £30-£40 per hour (Ltd company)
• On-site: Must be on-site (No remote working)
• Weekdays only (no weekends/evenings)

Role Overview:
We are looking for a Locum Independent Prescriber Pharmacist to join our healthcare team in Wycombe. This is a maternity leave cover position, offering flexibility in working hours (full-time or part-time) and is expected to last up to 12 months. You will play a key role in the Long-Term Conditions (LTC) team, providing comprehensive reviews and ongoing management of patients with chronic diseases.

Key Responsibilities:

• Conduct holistic reviews of patients with long-term conditions such as:
• Hypertension
• Diabetes
• COPD
• Asthma
• Lipid control
• Chronic kidney disease
• Heart failure
• Prescribe and manage treatment plans as an independent prescriber.
• Collaborate with the multidisciplinary healthcare team to deliver high-quality patient care.
• Provide expert advice on medication and treatment to patients with chronic conditions.
• Ensure compliance with clinical guidelines and best practices.

Essential Qualifications & Experience:

• Independent Prescriber qualification (IP).
• Experience working in a pharmacy setting, ideally in a GP surgery or primary care.
• Strong knowledge of chronic disease management.
• Excellent communication and patient care skills.
• Ability to work independently and as part of a team.
