What You’ll Be Doing:

Join our team as a Discharge to Assess Physiotherapist and play a key role in supporting patients’ safe transition from hospital to home or care placements. In this role, you will:

• Undertake assessment and intervention in the community and care homes under the D2A pathways
• Assess patients’ therapy and care needs to promote safety and independence
• Provide interventions aimed at reducing risk of re-admission or harm, supporting patients onto the best pathway for their needs
• Coordinate services by completing person-centred assessments, developing and implementing care and therapy plans
• Assess equipment needs, support discharge planning, and set rehabilitation goals to optimise function and independence
• Work autonomously within professional guidelines and the service’s policies and procedures
• Assess for and arrange health or social care packages to meet patients’ needs (training provided as needed)


Requirements & Qualifications:

To be successful in this role, you will need:

✔ Current and valid HCPC registration
✔ Previous experience in community rehab, discharge to assess, or care home settings
✔ Strong communication and interpersonal skills
✔ Ability to work independently and within a multidisciplinary team
✔ Commitment to delivering high-quality, person-centred care
✔ Full UK driving licence and access to a vehicle for community visits


Working Hours & Contract Duration:

This role offers a flexible working position, requiring availability Monday to Friday, with flexibility where possible. The contract is ongoing with a strong possibility of extension based on performance and service needs.
