Join our team as a Community Physiotherapist and play a key role in supporting patients to regain independence and function within their own homes and community settings. In this role, you will:
• Perform comprehensive assessments of patients with a wide range of conditions
• Create and implement personalised care and rehabilitation plans
• Deliver evidence-based treatments to optimise patient outcomes and independence
• Coordinate care by liaising with families, carers, and wider multidisciplinary teams
• Assess equipment needs to promote safety and function at home
• Maintain accurate and timely patient records in line with professional standards
• Promote health and wellbeing to prevent deterioration or hospital admission
To be successful in this role, you will need:
✔ A current and valid HCPC registration
✔ Experience working with adults in community rehabilitation or acute settings
✔ Strong communication and interpersonal skills
✔ The ability to work independently and within a multidisciplinary team
✔ Commitment to delivering high-quality, person-centred care
✔ A full UK driving licence and access to a vehicle for community visits
This is a permanent full-time position, working Monday to Friday, with flexibility where needed. Salary ranges from £30,000 to £50,000 per annum, depending on experience.
