We are currently seeking a proactive and compassionate Occupational Therapist to join our dynamic private healthcare service in Surrey. This unique and rewarding role involves working closely with patients in the Accident & Emergency (A&E) department, conducting initial assessments, and providing seamless support as they transition back to their home environment.
As part of a service that bridges the gap between acute and community care, you will play a key role in delivering person-centred interventions that promote safety, independence, and recovery.
Conduct timely and holistic occupational therapy assessments within the A&E setting.
Identify patients suitable for discharge and provide strategies or interventions to support safe return home.
Continue assessments and provide follow-up visits in patients’ homes to evaluate function and implement appropriate support plans.
Collaborate with hospital and community-based teams, including physiotherapists, social workers, and GPs, to ensure continuity of care.
Provide equipment recommendations, adaptations, and advice on risk management in the home environment.
Keep accurate and up-to-date clinical records in line with professional and legal standards.
Empower patients and families through education and support to maintain independence.
Degree or Diploma in Occupational Therapy
HCPC registration (Health and Care Professions Council)
Proven experience in either acute or community settings (experience in both preferred)
Excellent communication, organisational, and clinical reasoning skills
Ability to work autonomously and manage a varied caseload
Access to own vehicle and full UK driving license (essential for community visits)
