The Band 6 Urgent Community Response (UCR) Clinician plays a vital role in delivering rapid, person-centred care to patients in their own homes or usual place of residence, helping to prevent unnecessary hospital admissions and supporting timely discharges.
Working as part of a dynamic, multidisciplinary team (MDT), the post holder will assess, plan, and implement high-quality care for adults experiencing an acute episode of illness or functional decline that can be safely managed in the community within a two-hour response timeframe.
You will use advanced clinical skills, sound clinical judgement, and evidence-based practice to support patients’ recovery, maximise independence, and promote self-management in line with NHS England’s Urgent Community Response framework.
Undertake rapid, holistic assessments of patients referred to the UCR service within the mandated 2-hour response window.
Formulate and implement safe, effective treatment and care plans in collaboration with the wider MDT, including GPs, therapists, social care, and voluntary sector partners.
Deliver high-quality interventions such as clinical monitoring, medication management, wound care, mobility support, and short-term rehabilitation.
Recognise and act appropriately on clinical deterioration, escalating concerns to senior clinicians, GPs, or emergency services as required.
Contribute to the prevention of unnecessary hospital admissions and support early discharge planning.
Maintain accurate, timely, and comprehensive patient records in accordance with local and national standards.
Provide clinical leadership, supervision, and mentorship to junior staff, students, and support workers.
Contribute to service development and quality improvement initiatives within the UCR team.
Participate in clinical audits, governance meetings, and multidisciplinary reviews to promote learning and service excellence.
Build effective working relationships with primary care, community nursing, therapy teams, social services, and voluntary organisations to ensure coordinated, patient-centred care.
Communicate sensitively and effectively with patients, families, and carers about care plans and goals of treatment.
Promote the UCR service across local health and care systems to support awareness and appropriate referrals.
