Hunter Gatherer AHP are currently recruiting for a Locum Community Rehabilitation Occupational Therapist to join a supportive community rehabilitation service based in Cheshire.
This is a fantastic opportunity for an Occupational Therapist who enjoys working within the community and supporting patients to regain their independence and remain safely in their own homes.
You will work as part of a multidisciplinary community rehabilitation team, supporting patients with a range of rehabilitation needs.
Key responsibilities include:
Completing Occupational Therapy assessments within patients' homes and community settings
Supporting patients to regain and maintain independence
Developing and implementing personalised rehabilitation plans
Assessing functional ability and equipment needs
Supporting safe discharge from hospital
Helping to prevent unnecessary hospital admissions
Working closely with Physiotherapists and other healthcare professionals
Managing a caseload and maintaining accurate clinical documentation
Qualified Occupational Therapist with relevant UK experience
HCPC registration
Experience in community rehabilitation, intermediate care, discharge planning, or rehabilitation
A full UK driving licence and access to a vehicle is essential for this role
Ability to work independently within the community
