• Rate of Pay: Between £30 and £32 per hour based on experience.

• Performing comprehensive patient assessments with a tailored approach.
• Creating and implementing personalised care plans.
• Delivering evidence-based treatments to optimise patient outcomes.
• Collaborating with multidisciplinary healthcare teams for holistic care.
• Maintaining detailed and accurate patient records for compliance.

Requirements & Qualifications:

• Current and valid HCPC registration (or relevant governing body registration).
• Experience working with diabetes patients in the community.
• Clinical NHS experience at band 7.
• Strong communication and interpersonal skills.
• Ability to work both independently and as part of a multidisciplinary team.
• Commitment to delivering high-quality patient care.

Working Hours & Contract Duration:

This role offers a flexible working position requires availability from Monday to Friday with flexibility where possible. The contract is for an initial 3 months with a strong possibility of extension based on performance and service needs.
