We are seeking a proactive and compassionate Band 6 Occupational Therapist to join a dynamic A&E and Rapid Response Team in Surrey. This is an exciting opportunity for an experienced OT to work in a fast-paced, multidisciplinary environment focused on early intervention, admission avoidance, and timely discharge planning.
The post holder will provide specialist assessment and intervention for patients presenting in A&E or referred through the community rapid response pathway, supporting safe transitions of care and maximising independence.
Provide comprehensive functional assessments and rapid interventions for patients in A&E, acute wards, and community settings.
Work closely with the multidisciplinary team (including physiotherapists, nurses, social workers, and medical staff) to facilitate safe and effective discharges.
Support admission avoidance initiatives, assessing patients at home or in the community to prevent unnecessary hospital admissions.
Develop and implement individualised care plans promoting independence and person-centred goals.
Provide equipment recommendations, arrange community services, and liaise with carers and families.
Contribute to service development within the rapid response pathway.
Maintain accurate and timely clinical records in line with professional standards.
HCPC registration as an Occupational Therapist.
Degree or diploma in Occupational Therapy.
Minimum 2 years post-qualifying experience, ideally in A&E, acute medicine, or community/rapid response settings.
Excellent communication, clinical reasoning, and time management skills.
Ability to work autonomously and as part of a multidisciplinary team.
Car driver with access to a vehicle (essential for community work).
