Join our established Community Mental Health Team and support adults with complex mental health needs by:
Undertaking comprehensive assessments in the community and clinic settings
Developing and implementing personalised, recovery-focused interventions
Supporting service users to develop skills for independent living and social inclusion
Working closely with psychiatrists, social workers, CPNs and other professionals to deliver holistic care
Contributing to care planning, risk assessments and discharge planning
Maintaining accurate and timely clinical documentation
To be successful in this role, you’ll need:
A current and valid HCPC registration
Previous experience in mental health (community setting desirable)
Strong knowledge of mental health conditions, recovery models, and risk management
Excellent communication and teamwork skills
A commitment to delivering person-centred, evidence-based care
