We are currently recruiting for a Locum Community Occupational Therapist to join a supportive NHS community team in Cheshire.

This is a great opportunity for an experienced OT seeking part-time work within a well-established community service.

Location: Cheshire
Setting: NHS Community Team
Start Date: ASAP
Days: 2 days per week
Rate: £25 – £26 per hour

The Role:

You will be managing a community caseload, supporting adults in their own homes to maximise independence, safety, and functional ability. The role involves completing holistic assessments and implementing appropriate interventions to facilitate safe discharge and prevent hospital admissions.

Key Responsibilities:

• Completing functional and environmental assessments in patients’ homes
• Assessing for and recommending equipment and minor adaptations
• Supporting hospital discharge and admission avoidance pathways
• Developing and reviewing person-centred care plans
• Providing moving and handling assessments and advice
• Liaising with MDT colleagues, carers, and external agencies
• Maintaining accurate and timely clinical documentation in line with NHS standards

Requirements:

• HCPC registered Occupational Therapist
• Previous NHS community experience
• Experience with equipment provision and home adaptations
• Full UK driving licence and access to a vehicle
• Strong assessment and clinical reasoning skills
