Join our team as a Community Rehabilitation Occupational Therapist and play a key role in improving the lives of adults in their own homes and community settings. In this role, you will:
• Perform comprehensive functional and environmental assessments
• Create and implement personalised rehabilitation and care plans
• Deliver evidence-based interventions to optimise patient safety, function, and independence
• Assess for and recommend adaptive equipment and minor adaptations
• Work collaboratively with multidisciplinary teams to provide holistic care
• Maintain accurate and timely patient records in line with professional standards
To be successful in this role, you will need:
✔ A current and valid HCPC registration
✔ UK experience working as an Occupational Therapist in community rehabilitation settings
✔ Strong communication and interpersonal skills
✔ Ability to work independently and within a multidisciplinary team
✔ Commitment to delivering high-quality, person-centred care
✔ Full UK driving licence and access to a vehicle for community visits

This role offers a flexible working position, requiring availability Monday to Friday, with part-time options considered. The contract is ongoing with a strong possibility of extension based on performance and service needs.
