£27–£29 per hour | Locum | General Community

We are currently recruiting for a Community Occupational Therapist to join a well-established general community team in South East London. This is an excellent opportunity for an experienced OT looking for a rewarding locum role with competitive hourly rates.

The Role

As a Community Occupational Therapist, you will be working within a multidisciplinary team providing high-quality, person-centred assessments, rehabilitation, and interventions to adults within their own homes and community settings.

Key Responsibilities

  • Undertake comprehensive functional assessments in patients’ homes
  • Provide rehabilitation programmes to maximise independence and functional ability
  • Support patients following hospital discharge with community-based rehabilitation
  • Assess for and recommend appropriate equipment and minor/major adaptations
  • Complete moving and handling assessments
  • Develop and implement goal-focused treatment and rehabilitation plans
  • Carry out risk assessments and safeguarding where required
  • Liaise effectively with families, carers, and other healthcare professionals
  • Maintain accurate and timely clinical records
  • Support hospital discharge and prevent unnecessary admissions where applicable

Requirements

  • HCPC registered Occupational Therapist
  • Previous community and/or rehabilitation experience
  • Strong assessment, rehabilitation, and care planning skills
  • Ability to work independently and manage a caseload
  • Full UK driving licence (preferred for community visits)
