We are currently recruiting an experienced Locum Band 6 Occupational Therapist to join a community Service in South Wales. The role is to start ASAP and is 3 days per week.

Role Overview:
The Occupational Therapist will work closely with the multi-disciplinary team to respond to referrals for patients who are likely to be admitted to hospital. This Service delivers clinic-based and home-based assessments and interventions, focusing on admission avoidance for frail older adults approaching crisis.

The service also includes a Bridging function, supporting patients with care packages until Local Authority capacity is available, helping to maintain patient flow from acute hospital sites. The OT will be expected to support colleagues with therapy escalation for patients on this pathway.

Key Responsibilities:

  • Rapid response to assessment requests, including same-day assessments
  • Home environment reviews and risk management
  • Home-based assessment and intervention, including crisis management of urgent manual handling needs
  • Caseload management and positive risk management to prevent avoidable admissions
  • Supporting workflow and patient flow across the service
  • Providing equipment and liaising with community services for onward rehabilitation and environmental adaptations
  • Working with Local Authority and third-sector organisations to address longer-term needs
  • Participating in MDT triage to prioritise referrals and action work accordingly

Essential Requirements:

  • Band 6 Occupational Therapy experience
  • Experience of discharge planning and working across community/inpatient pathways
  • Ability to undertake rapid assessments
  • Car driver with access to a vehicle

Desirable:

  • Experience using RIO system

Location: South Wales
Start Date: ASAP
Hours: 3 days per week
