Join our supportive South Wiltshire reablement team as a Locum Reablement Occupational Therapist and make a real impact in the community. In this role, you will:
Work with adults to regain independence and confidence after illness, injury, or hospital discharge
Carry out holistic and comprehensive functional assessments within service users’ homes or community settings
Develop and deliver tailored reablement plans focused on maximising independence and reducing ongoing care needs
Collaborate with multidisciplinary teams including social workers, physiotherapists, carers, and voluntary sector partners
Monitor progress, evaluate outcomes, and adapt plans to ensure safe, effective support
Maintain accurate patient and service records in line with HCPC standards
To be successful in this role, you will need:
A current and valid HCPC registration
Previous experience in reablement, community OT, or hospital discharge settings (desirable)
Strong communication and interpersonal skills
The ability to work independently and as part of a multidisciplinary team
A passion for promoting independence and improving quality of life
This role offers full-time hours with flexibility around start and finish times to suit your schedule. The contract is ongoing with a strong possibility of extension based on performance and service needs.
