Join our team as an Independent Prescribing Pharmacist and play a key role in improving the lives of patients in a GP surgery setting. In this role, you will:
• Perform comprehensive patient assessments
• Create and implement personalised care plans
• Deliver evidence-based treatments to optimise patient outcomes
• Collaborate with multidisciplinary teams to provide holistic care
• Maintain accurate patient records
To be successful in this role, you will need:
• A current and valid GPhC registration as an Independent Prescriber
• Experience working in primary care settings using EMIS and SystemOne
• Strong communication and interpersonal skills
• The ability to work independently and within a multidisciplinary team
• A commitment to delivering high-quality patient care
This full-time role requires availability Monday to Friday, 9–5, with a 30-minute lunch. The contract is ongoing with a strong possibility of extension based on performance and service needs.
