Join our client’s GP surgery service as a Diabetes Pharmacist and play a key role in supporting patients with sub-optimally controlled diabetes. In this role, you will:
run dedicated diabetes follow-up clinics
review recent blood results and patient records
up-titrate diabetes medications where clinically appropriate
provide structured medication reviews and optimise therapy
offer lifestyle and adherence support
maintain accurate clinical documentation
This is a fully remote role offering excellent flexibility — 1–2 days per week (any day), with up to 4 half-day sessions (4 hours per session, between 9am–5pm).
To be successful in this role, you will need:
GPhC registration
solid experience managing diabetes in primary care or similar setting
confidence in medication optimisation and dose titration
SystmOne (S1) access on your own PC for remote work
a valid NHS Smart Card
strong communication skills and ability to work autonomously
Please note: S1 access and Smart Card are essential for this remote position.
This role offers flexible working and requires availability for 1–2 days per week (maximum 4 sessions weekly). The contract is initially for 3 months with potential for extension depending on service needs.
