PCN-Experienced Pharmacist – GP Surgery – Hampshire – £26–£34/hour (DOE)

Hunter AHP have an excellent opportunity for a Clinical Pharmacist with PCN experience to join a forward-thinking GP surgery in Hampshire. This is a long-term full-time locum post, ideal for a pharmacist looking to make a real clinical impact within a supportive and collaborative primary care team.
This role is based within a large, multi-GP surgery that is part of a wider Primary Care Network. You’ll be working alongside GPs, nurses, and a dedicated medicines optimisation team to deliver high-quality care to a diverse patient population.
You’ll be expected to manage your own clinical workload, contribute to service development, and deliver patient-facing clinics. While independent prescriber (IP)status is highly desirable, we are also welcoming applications from non-prescribing pharmacists with strong PCN or GP experience.
Key Responsibilities:
Conduct Structured Medication Reviews (SMRs)and follow-ups
Support long-term condition management (e.g., hypertension, asthma, diabetes)
Manage repeat prescriptions, medicines reconciliation, and deprescribing
Provide medicines-related adviceto clinicians and patients
Implement medicines optimisation initiatives and cost-saving audits
Assist with QOF and IIF targets, improving prescribing safety and outcomes
Use EMIS or SystmOneto maintain accurate patient records and documentation
The Ideal Candidate Will Have:
GPhC registration as a Pharmacist
Minimum 6–12 months’ experience in a PCN or GP setting
Strong working knowledge of EMIS or SystmOne (required)
Excellent communication, clinical judgment, and patient counselling skills
Independent Prescriber status (preferred but not essential)
Commitment to full-time availability (Monday to Friday, 9am–5pm)

Rate: £26 – £34 per hour (based on experience & qualifications)
