JOB DESCRIPTION

Hunter Gatherer AHP are currently recruiting for an experienced Band 6 Senior Occupational Therapist to join a Discharge to Assess service in South Essex.

This is an excellent opportunity for an experienced OT looking for an ongoing locum position, working 4 days per week / 30 hours per week, Monday–Friday.

The successful candidate will undertake therapy assessments within the community, visiting patients in their own homes following discharge from hospital. The role will focus on facilitating safe transfers home and supporting ongoing pathway decisions.

KEY RESPONSIBILITIES

  • Undertake comprehensive OT assessments in patients' own homes following hospital discharge.

  • Assess patients' functional abilities and ongoing therapy needs.

  • Support safe discharge and transfers from hospital into the community.

  • Make recommendations regarding appropriate equipment, adaptations and ongoing support.

  • Contribute to pathway and care planning decisions.

  • Work collaboratively with the wider multidisciplinary team.

  • Maintain accurate clinical documentation and provide high-quality patient-centred care.

ROLE DETAILS

  • Band 6 Senior Occupational Therapist

  • Discharge to Assess

  • South Essex

  • 4 days per week / 30 hours

  • 8:30am–4:30pm

  • Monday–Friday

  • ASAP start

  • Ongoing with possible extensions

REQUIREMENTS

  • HCPC registered Occupational Therapist.

  • Previous Band 6 experience.

  • Community/discharge experience desirable.

  • Experience completing functional assessments in patients' homes.

  • Strong clinical reasoning and communication skills.

  • Ability to work independently within a community setting.

  • Full UK driving licence and access to a vehicle required.
