What You’ll Be Doing:

Join our team as a Band 6 as A discharge-to-assess occupational therapist in a care home setting.

The main responsibilities include:

  1. Assessing Readiness for Discharge: The therapist evaluates a patient’s ability to safely leave the healthcare facility and return to their home or community environment. This involves assessing their functional capabilities and safety in activities of daily living (ADLs), such as dressing, bathing, cooking, and mobility.
  2. Coordinating Care Post-Discharge: They work to ensure continuity of care after discharge. This may involve coordinating with community services or home health agencies to provide ongoing support, therapy, or assistance at home.
  3. Providing Recommendations: Based on their assessment, the therapist may suggest modifications to the patient’s home environment (e.g., recommending grab bars in bathrooms or assistive devices). They also provide advice about ongoing therapy needs, such as outpatient occupational therapy.
  4. Patient and Caregiver Education: They educate both the patient and their caregivers on strategies and techniques to facilitate independence at home, prevent further injury, and manage any challenges the patient might face.
  5. Evaluating Mental and Emotional Readiness: Apart from physical functioning, the therapist assesses the mental and emotional readiness of the patient for discharge, identifying any concerns related to coping, anxiety, or depression, which may require further attention.
  6. Collaborating with the Healthcare Team: The discharge-to-assess occupational therapist works closely with doctors, nurses, social workers, and other professionals to create a comprehensive discharge plan that ensures the patient’s safety and well-being post-discharge.

In short, they ensure that the patient is physically, emotionally, and logistically prepared to leave the hospital or rehab setting and transition smoothly back to their daily life.

Requirements & Qualifications:

To be successful in this role, you will need:

  • a current and valid HCPC registration
  • experience working with Children in an acute setting
  • strong communication and interpersonal skills
  • the ability to work independently and within a multidisciplinary team
  • a commitment to delivering high-quality patient care 

 

Working Hours & Contract Duration:

This role offers a flexible working position requires availability from Monday – Friday with flexibility where possible. The contract is ongoing with a strong possibility of extension based on performance and service needs.
