Hunter Gatherer AHP is looking for an experienced Band 6 Physiotherapist to join a busy Acute Medicine Therapy Team based in Leicester.
The successful candidate will have experience working within acute inpatient services, providing comprehensive physiotherapy assessments, treatment, rehabilitation, and discharge planning for patients with a range of acute medical conditions. You will work closely with the multidisciplinary team to facilitate safe, effective, and timely patient discharges while promoting independence and preventing hospital deconditioning.
Key Responsibilities:
Assess and treat patients within an acute medical inpatient setting.
Develop and implement individualised rehabilitation programmes.
Support complex discharge planning and liaise with community services.
Work collaboratively within the multidisciplinary team.
Maintain accurate clinical documentation and adhere to HCPC and CSP standards.
Requirements:
HCPC registered Physiotherapist.
Previous Band 6 experience within an acute inpatient setting.
Strong acute medicine and discharge planning experience.
Excellent communication and organisational skills.
Available to start ASAP.
