What You’ll Be Doing:
Join our team as a Pharmacist Independent Prescriber and play a key role in improving the lives of patients through clinical reviews and proactive health management. In this role, you will:
• perform comprehensive medication reviews
• manage hypertension and chronic disease screening
• create and implement personalised care plans
• collaborate with multidisciplinary teams to provide holistic care
• maintain accurate patient records using EMIS and SystmOne
Requirements & Qualifications:
To be successful in this role, you will need:
• a current and valid GPhC registration
• Independent Prescriber qualification
• experience in medicines reviews, hypertension, and patient screening
• strong communication and interpersonal skills
• the ability to work independently and within a team
• experience using EMIS and SystmOne
Working Hours & Contract Duration:
This role offers a flexible working position requiring availability Monday to Friday, with flexibility where possible. The contract is ongoing with a strong possibility of extension based on performance and service needs.
