Job Description

Hunter Gatherer AHP is looking for an experienced Band 6 Occupational Therapist to join the Inpatient Therapy Team for an NHS Hospital.

The successful candidate will work across a variety of inpatient wards, managing a diverse caseload of adult patients. You will be responsible for completing comprehensive occupational therapy assessments, supporting rehabilitation, and facilitating safe and effective discharge planning. Working closely with the multidisciplinary team, you will help patients maximise their independence and achieve the best possible outcomes.

Key Responsibilities:

  • Complete functional and cognitive assessments for adult inpatients.

  • Assess activities of daily living (ADLs) and develop person-centred treatment plans.

  • Recommend and provide equipment and adaptations to support safe discharge.

  • Carry out home assessments where appropriate.

  • Work collaboratively with the multidisciplinary team to coordinate discharge planning.

  • Liaise with patients, families, and community services to ensure seamless discharge pathways.

  • Maintain accurate clinical documentation in line with HCPC and Trust standards.

Requirements:

  • HCPC registered Occupational Therapist.

  • Previous NHS experience working within inpatient wards.

  • Experience with discharge planning and rehabilitation.

  • Strong assessment, communication, and clinical reasoning skills.

  • Ability to manage a varied caseload independently.

  • Available to start ASAP.
