We are currently recruiting for an experienced Band 6 Physiotherapist to join an Intermediate Care Team in Cambridgeshire.
The role involves working across care at home and care home settings, supporting patients to maximise independence, recovery and rehabilitation.
Manage a shared caseload across community and care home settings.
Complete holistic assessments of patients with complex needs and multiple pathologies.
Use specialist clinical reasoning to identify patient needs and agree appropriate rehabilitation goals.
Provide physiotherapy interventions and rehabilitation programmes.
Support patients to maximise independence and facilitate safe discharge.
Work in line with the Home First model, promoting recovery and reducing unnecessary hospital stays.
Undertake appropriate risk assessments and respond to changes in patient needs.
Work collaboratively with patients, carers, colleagues and other professionals.
Provide specialist physiotherapy advice regarding care plans, patient progress and ongoing rehabilitation needs.
Work autonomously while contributing effectively to the wider multidisciplinary team.
Maintain accurate and timely clinical records in line with HCPC requirements.
HCPC registered Physiotherapist
Previous experience in community, intermediate care or rehabilitation
Experience working with complex and multiple pathologies
Strong clinical reasoning and autonomous working skills
Experience developing and delivering rehabilitation programmes
Understanding of the Home First model
Strong communication and multidisciplinary working skills
Available to start ASAP
