Hunter Gatherer AHP are recruiting for a Band 6 Physiotherapist to join a Community Rapid Response service based in East London. This is an excellent opportunity to work within a fast-paced multidisciplinary team, providing urgent assessment and rehabilitation to help prevent unnecessary hospital admissions and support patients to remain safely at home.

The successful Physiotherapist will:

• Provide urgent physiotherapy assessment and treatment within community settings.

• Respond rapidly to referrals for patients experiencing acute functional or mobility decline.

• Assess mobility, balance, strength, transfers and falls risk.

• Deliver timely rehabilitation interventions to maximise independence.

• Develop immediate management plans to safely support patients at home.

• Work closely with the multidisciplinary team to deliver coordinated crisis response.

• Facilitate early supported discharge and help prevent hospital readmissions.

• Recommend appropriate equipment, exercise programmes and environmental adaptations.

• Educate patients and carers to promote self-management and rehabilitation goals.

• Maintain accurate clinical records and contribute to service development.

To be successful in this role, you will need:

• HCPC registration as a Physiotherapist.

• Previous NHS experience as a Band 6 Physiotherapist.

• Community and/or Rapid Response experience is highly desirable.

• Excellent assessment, rehabilitation and discharge planning skills.

• Full UK driving licence and access to a vehicle, as travel is required across North East London.

Working Hours: Full-Time | Monday – Friday
